Who Should Be on a HAZOP Team?
An effective HAZOP team is genuinely multidisciplinary: a trained HAZOP facilitator who runs the session and keeps the team disciplined about the methodology, a scribe who records deviations and actions in real time, process and instrumentation engineers who understand the design intent, operations personnel who know how the plant actually runs day to day (not just how it was designed to run), and often a maintenance representative. Facilitator independence matters – an external, experienced facilitator tends to surface issues that an internal team, familiar with the design, can overlook.
The HAZOP Process, Step by Step
1. Preparation
Gather current P&IDs, process descriptions, operating procedures, and prior incident data. Divide the process into logical nodes (sections with a consistent design intent) for systematic review.
2. The Study Sessions
For each node, the team applies guide words to each relevant process parameter, identifies credible deviations, discusses possible causes, evaluates consequences, and reviews existing safeguards. Where the team judges the risk insufficiently controlled, an action is recorded with an owner and a target date.
3. Documentation
Every deviation considered, whether or not it generates an action, is recorded – this creates a defensible, auditable record of the study and is often requested by regulators or auditors even years later.
4. Action Closure and Follow-Up
The real value of a HAZOP is realised only when recommended actions are tracked to closure. A HAZOP report full of open, unclosed actions is a liability, not an asset, in an audit or after an incident.
Common Mistakes That Undermine a HAZOP
The most frequent problems are rushing sessions to save time, which produces shallow analysis; using outdated P&IDs that do not reflect as-built conditions; excluding operations staff who understand real-world deviations from design assumptions; and, most damagingly, treating the final report as a compliance deliverable to file away rather than a live action list to close out.
